Westhab in Yonkers, NY is seeking a Full-time Case Manager for the Rapid Re-Housing (Permanent Supportive Program). You will perform intake, verify eligibility, and coordinate with DSS and housing staff to secure permanent housing for clients. The role requires a Bachelor or Associate in social work (or related field) and at least two years of direct client services experience; fluent Spanish is highly desirable.
