Key Responsibilities\n- \n
- Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, contributing as an integral member of the primary care team. \n
- Conduct in-depth patient intakes that emphasize diagnostic and functional evaluations, followed by recommending appropriate treatment goals and plans to primary care providers. \n
- Provide brief, focused interventions for patients requiring mental health services while regularly monitoring patient progress and adapting treatment plans as necessary. \n
- Initiate follow-ups to evaluate patient well-being and assess the need for adjustments in therapeutic efforts. \n
- Develop, when necessary, relapse prevention strategies and aid patients in maintaining stable functioning. \n
- Assist in identifying patients at risk and contribute to developing plans aiming to prevent the deterioration of their conditions. \n
- Link patients to essential behavioral health services, collaborating with external treatment providers when necessary and permitted by the patients. \n
- Maintain comprehensive documentation of patient progress, treatment goals, and necessary diagnostic information to keep healthcare providers informed. \n
- Promote healthier lifestyle choices among patients with physical disorders, employing behavioral change techniques as applicable. \n
- Offer patients educational resources and self-management skills to empower them in their recovery journey. \n
- Engage in consultations with clinic management regarding behavioral services and contribute ideas for improved program evaluation and outcomes. \n
- Support patients in adhering to medical recommendations from primary care providers, including strategies for managing medication side effects. \n
- Adhere strictly to the Code of Ethical Conduct for Clinical Professionals, ensuring the highest standards of patient care. \n
- Carry out other duties as assigned by management to ensure the operational effectiveness of the clinic. \n
As a Licensed Professional Counselor at Thrive Hard, you will work with a diverse range of patients, including individuals facing anxiety, depression, trauma, and other mental health challenges. The clinic's integrated approach facilitates an environment where patients can receive holistic care that encompasses both physical and mental health services, making each interaction an opportunity for meaningful impact.
Qualifications\n- \n
- Master's Degree in Clinical Counseling or a related field is required. \n
- Active LPC (Licensed Professional Counselor) license in Pennsylvania. \n
- Comprehensive knowledge of mental health assessment, treatment planning, and various therapeutic modalities. \n
- Understanding of the developmental psychopathology and group therapy, particularly as it relates to family and child therapy. \n
- Familiarity with DSM-IV diagnoses and proficiency in behavioral therapy techniques. \n
- Strong communication skills, with bi-lingual proficiency in English and Spanish preferred but not mandatory. \n
- Must possess the mental and physical capacity to meet the demands of counseling responsibilities. \n
- Solid understanding of the societal impacts of mental and physical health issues on individuals, families, and the broader community. \n
- Extensive knowledge of community resources and how they can be utilized to support patient care. \n
- Experience in psychodiagnostics and psychotherapeutic methods. \n

- Prior experience working in a community health setting or integrated behavioral health environment. \n
- Familiarity with Medicaid and Medicare guidelines as they relate to service delivery. \n
- Track record of achieving productivity targets in a clinical setting. \n
- Experience with electronic health record (EHR) systems for documentation and case management. \n
The salary for this position ranges from USD 87,000 to 105,000 per year, commensurate with experience and qualifications.
