Memory Care Coordinator Licensed Practical Nurse position

Responsibilities Plan and coordinate a therapeutic program meeting spiritual, social, emotional, physical, and intellectual needs of residents Assess resident characteristics and, with other departments, plan and organize program content Monitor daily functioning, ensuring continuity and appropriate changes Assist in developing, implementing, and conducting in‑service training for staff on memory care programs Establish teamwork through communication, interaction, and meetings; maintain relationships with staff and integrate services Participate in support groups as directed by the Life Enrichment Director Assess educational needs of staff and collaborate with Life Enrichment Director for education Market program via community organizations and Alzheimer’s associations Maintain accurate documentation in compliance with state regulations and community policy Work with management to develop written program objectives and procedures for implementation and evaluation Serve as a role model in dementia care Perform pre‑admission assessment of potential residents Assist with admission transition Assist adjustment of residents and families, contacting weekly during the first month post‑admission Monitor current research, programs, and resources; refer to appropriate services to support residents’ social functioning Assist residents with maintenance and supply of personal clothing and other items Refer resident sponsors to internal and external services Maintain confidentiality of user ID code and report any violations Preferred Skills and Qualifications LPN credential Experience with Alzheimer’s and other dementia individuals At least two years of programming experience in assisted living, including POC, scheduling, coordinating meetings with POAs and families, and planning activities Preference for three to five years of experience Background in nursing or providing one‑on‑one care for seniors Flexible schedule, willing to work evenings, weekends, holidays, and on‑call rotations EEO Statement Danbury Senior Living is an Equal Opportunity Employer and considers all applicants for positions without regard to race, color, religion, sex, national origin, age, sexual orientation, marital or veteran status, or non‑job‑related handicap or disability. #J-18808-Ljbffr
