Pharmacy Duration: 13 weeks Shift: 8-hour shifts Hours per Shift: Start and end times may vary, likely to include: 6:00 AM - 2:30 PM, 7:00 AM - 3:30 PM, 8:00 AM - 4:30 PM, 11:00 AM - 7:00 PM, or 3:00 PM - 11:00 PM Experience: Minimum 1 year preferred License: Pharmacy state licensure required Certifications: BLS or ACLS required; PALS required (for pediatric practice) Must-Have: Doctor of Pharmacy or BS Pharmacy degree required Description: All pharmacists fill orders for drugs, monitor patient drug therapies, and provide drug information. Pharmacists deliver pharmacy services with compassion, excellence, and efficiency. They prepare and dispense drug orders per physician request according to established policies, procedures, and protocols. Pharmacists interpret drug orders (verbal and written) and transcribe/verify them into computerized patient medication records accurately. They maintain accurate and complete patient drug records. Pharmacists compound and dispense pharmaceuticals, including non-sterile and sterile products, chemotherapy, and parenteral nutrition products accurately. They issue controlled substances to patient care areas and maintain records as required by law and institutional policies. Pharmacists provide services efficiently and in a timely manner. They maintain their current pharmacist license and ensure applicable continuing education records and licensure are maintained in department files. Pharmacists attend staff meetings and complete all competency/skills assessment requirements. They supervise and direct pharmacy support personnel and verify the daily activities of pharmacy technicians. Ambulatory care pharmacists provide pharmaceutical care services utilizing approved protocols and/or collaborative agreements with other providers. They manage patient drug therapy, anticoagulation management, and patient education, as well as overseeing medication preparation and dispensing. Pharmacists perform duties in clinical areas such as anticoagulation clinics, intensive medical homes, transition of care, and discharge medication reconciliation. They ensure safe, appropriate, cost-effective drug therapies for patients according to established policies, procedures, and protocols. Pharmacists monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose. They assist with pharmacokinetics consult services and renal drug dosing per hospital protocol. Pharmacists accurately read, extract, and interpret information in patient medical records. They detect and report suspected adverse drug reactions in a timely manner. Pharmacists sustain the hospital drug formulary, minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection. They provide clinical consultation and clarification to practitioners as appropriate and accurate drug information to the professional staff. Pharmacists provide drug education to patients and their families per institutional protocol. They participate in quality improvement and medication use review activities of the department. Pharmacists collect data, conduct monitors and inspections, and maintain logs, records, and other documentation as assigned. They conduct designated interventions as defined in department clinical intervention activities. Pharmacists participate in antibiotic stewardship, opioid stewardship, and shortage management activities as assigned. They attend interdisciplinary rounds when assigned and provide services efficiently and in a timely manner. Pharmacists may face potential exposure to hazardous and toxic substances, sticks or cuts by needles and other sharp items, and musculoskeletal injuries if proper lifting techniques are not used. There is potential exposure to biologic material when participating in direct patient care activities.
