RN Case Manager II

Home Health Services of Virginia
The Case Manager II is responsible for maintaining a caseload of patients, completing all oversight related to patient care by other nurses and coordination with the Director of Nursing to ensure compliance and standards of care are met. We are looking for individuals that are dedicated and dependable. The Case Manager II must have the following:
- Must hold a current license as a Registered Nurse from the Virginia Board of Nursing.
- Must have experience working in a home care agency.
- Must have direct patient care experience.
