What You'll Do

• Be a voice for residents. Advocate for their rights, dignity, and well-being.
• Build meaningful relationships. Connect with residents, families, and care teams.
• Make transitions easier. Guide residents and families through move-ins, care planning, and changes in care.
• Lead with compassion. Provide emotional support, resources, and solutions.
• Make an impact. Coordinate personalized care plans, assessments, and social services while ensuring regulatory compliance.

 

What You Bring

• Master's in Social Work + 1 year of healthcare experience OR
• Bachelor's in Social Work, Psychology, Sociology, or related field + 3 years of healthcare experience
• At least 1 year of supervised healthcare experience
• Social Worker license as required by state regulations
• CPR certification
• A passion for helping older adults live their best lives!
