Your Responsibilities

• Conducting thorough patient evaluations, prescribing diagnostic tests, and crafting personalized care regimens
• Diagnosing and managing acute, episodic, or chronic health conditions collaboratively with physicians or autonomously (according to your credentials)
• Providing education to patients and their families on wellness practices, prevention, and disease management
• Prescribing medications and therapies within your practice parameters
• Engaging in patient follow-ups and chronic care management
• Collaborating with interdisciplinary teams to guarantee consistent, comprehensive care
• Accurately documenting in electronic medical records (EMR)

Requirements

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Graduation from an accredited Nurse Practitioner program

Certifications Needed

• Active Nurse Practitioner licensure in the state of New York
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or alternative specialty credential
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or capability to achieve within 90 days)
