Registered Nurse Case Manager We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at Willowbrook Hospice in Cartersville, Georgia. As a Registered Nurse Case Manager, you will play a vital role in providing exceptional care to our patients and their families. About the Role As a Registered Nurse Case Manager, you will be responsible for coordinating patient care and communicating with patients, families, and healthcare providers. You will assess patient needs, develop and implement individualized care plans, and provide ongoing support to patients and their families. Your expertise in hospice care will ensure that our patients receive the highest level of care and support, addressing their physical, emotional, social, and spiritual needs.
