Licensed Practical Nurse

Concordia is seeking compassionate and dedicated Licensed Practical Nurse to provide high-quality nursing care and support to our residents. This role is responsible for administering medications, coordinating resident care, supervising nursing assistants, and ensuring residents receive exceptional care in a supportive environment.

Current openings: We are currently hiring for multiple Full Time and Part Time positions. We are looking for a Full Time LPN on First and Second Shift.

• First Shift Hours:6:30am- 2:30pm
• Second Shift Hours: 2:30pm-10:30pm
• Third Shift Hours: 10:30pm-6:30am

*Shift Differentials include: Second Shift, $3.00, Third shift, $2.00, and the weekend $3.00. These differentials are stackable!

Responsibilities:

• Administer medications in accordance with physician orders, policies, and regulations
• Complete accurate and timely resident assessments and documentation
• Monitor resident conditions and communicate significant changes to physicians and families
• Carry out physician orders, referrals, and treatment plans
• Provide treatments and nursing procedures as needed
• Supervise and support Nurse Aides by monitoring care quality, documentation, staffing coverage, and daily assignments
• Participate in shift report communication to ensure continuity of care
• Assist with direct resident care as needed
• Maintain compliance with HIPAA, safety standards, and state and federal regulations

Qualifications:

• Current Pennsylvania LPN license required; Or multi-state license
• Compassionate, caring, and resident-focused approach
• Strong leadership and communication skills
