Case Manager – Case Management

Typical schedule: Sunday–Thursday (subject to change based on staffing). Weekend requirement: Every other weekend. Holiday requirement: Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).

Caseload: 20–23 patients (unit-based). Daily census (reported): ~25 patients.

Minimum: 1 full year of acute care hospital experience required. Travel experience: Required — NO first-time travelers. Licensure: LCSW / MSW credentials required. Required certifications: BLS. Epic charting system required.

Conduct initial assessments within 24 hours of patient admission. Develop and coordinate discharge planning in collaboration with the medical team. Facilitate care coordination for post-acute care, durable medical equipment (DME), and facility placements. Work closely with insurance case managers and utilization management teams for prior authorizations and referrals. Provide biopsychosocial assessments and develop/revise the plan of care as necessary.

Must be licensed upon start date; pending licenses accepted for submission but final license required at start. Submission requirements before clinical review: interview availability, DOB & full SSN confirmed, completed skills checklist. Modules: Pre-start modules average 16 hours (non-billable); >20 hours requires approval. Local rule: 50-mile radius; local hires accepted at a lower bill rate. Must commit to 3 of 5 listed holidays; standard return-to-office and floating expectations apply.
