Responsibilities:

• Provide direct patient care including wound care, medication administration, and vital sign monitoring in home settings
• Assess and document patient health status, progress, and changes in condition accurately and in a timely manner
• Administer prescribed medications and treatments in accordance with physician orders and care plans
• Collaborate with RNs, physicians, and interdisciplinary care teams to ensure continuity of patient care
• Educate patients and family members on care plans, medications, and health management techniques
• Maintain accurate and up-to-date patient records in compliance with state and federal regulations
• Monitor patients for adverse reactions and report significant changes to supervising RN or physician promptly

Requirements:

• Active Licensed Practical Nurse (LPN) license in the state of Maryland
• Prior home healthcare, long-term care, or clinical nursing experience preferred
• Strong knowledge of nursing procedures, medication administration, and patient assessment
• Current CPR/BLS certification required
• Excellent communication, documentation, and interpersonal skills
• Ability to work independently and manage time effectively in a home care setting
• Reliable transportation and valid driver's license to travel to patient homes in the Baltimore area
