RN Care Manager: Discharge & Transitions Lead

A healthcare organization in Fort Wayne, Indiana is seeking a Registered Nurse Care Manager to coordinate and oversee discharge planning and case management activities. The ideal candidate will have 2-4 years of clinical nursing experience and proficiency in case management principles. Responsibilities include collaborating with interdisciplinary teams, developing discharge plans, and maintaining documentation. This is a PRN position with a flat rate for hours worked.
