Outreach Executive I

Oak Street Health seeks an Outreach Executive I to connect Medicare-eligible adults in underserved communities with our innovative primary care centers. In this patient-centered role, you will conduct community-based and phone outreach, educate prospective patients on preventative care, and assist with scheduling and onboarding. Working closely with clinical and care teams, you’ll help reduce barriers to access, support retention, and maintain accurate engagement records.
