8-Hour & 12-Hour Shifts Available

Position Summary

The Registered Nurse (RN) will provide skilled nursing services to patients in their homes in accordance with physician orders and the patient's individualized plan of care. The RN will perform assessments, coordinate care, provide patient education, and support patients in maintaining optimal health and independence.

Responsibilities

• Provide skilled nursing care in accordance with the patient's plan of care
• Complete comprehensive nursing and psychosocial assessments
• Monitor patient conditions and document clinical findings accurately
• Administer treatments and nursing interventions as ordered
• Provide medication management and patient education
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Promote patient health, safety, and disease prevention
• Educate patients and family members regarding diagnoses, medications, and treatment plans
• Complete all required documentation in a timely and professional manner

Qualifications

• Graduate of an accredited School of Nursing
• Current Pennsylvania Registered Nurse (RN) license in good standing
• Minimum of one (1) year of nursing experience preferred
• Home health, skilled nursing, or hospital experience preferred
• Knowledge of Medicare and Medicaid guidelines preferred
• Strong clinical assessment and documentation skills
• Excellent organizational and time management abilities
• Ability to communicate effectively with patients, families, physicians, and staff

Requirements

• Valid driver's license and reliable transportation
• Ability to travel to patient homes within the service area
• Ability to perform physical nursing duties including walking, standing, lifting, bending, and assisting patients as needed
• Must communicate effectively in English, both verbally and in writing
