Job Description

Job Title: Registered Nurse – Hospice Home Health

Location: Lawrence, MA

Rate: Up to $2,200 Gross Weekly + benefits available from day 1

Shift: Days, 8:00 AM – 5:00 PM, Monday–Friday

Contract: 13 weeks

Hours: 40 hours/week

Requirements:

Minimum 1 year of recent hospice experience
Recent hospice experience required; first-time travelers not accepted
Active, unencumbered Massachusetts RN license
Reliable transportation for home visits
Current driver's license
Ability to travel between patient homes

Certifications:

AHA BLS

Responsibilities:

Provide skilled hospice nursing care in patients' homes according to physician orders and care plans
Assess patients and provide appropriate nursing interventions
Complete hospice visits, including Start of Care and follow-up visits
Monitor patient conditions and communicate changes to the appropriate care team
Provide education and support to patients and their families
Complete accurate and timely clinical documentation
Collaborate with physicians and the interdisciplinary care team
