Commitment to unlocking the full potential of vulnerable seniors.Description: The Case Manager is a member of the Interdisciplinary Care Team. They will work in collaboration with the team to deliver and manage all aspects of participant care. The Case Manager will conduct required assessments and develop and follow individualized participant Care Plans. Address patient needs, communicate with participants and their families, advocate for participant needs to be met. In partnership with the Primary Care Provider, the Case Manager is responsible for delivering care in various settings, managing participant and family care needs. Take ownership of the outcomes and quality of care for their assigned participant panel, working directly with the Primary Care Provider. Conduct in-clinic, at home, and virtual assessments, educating participants and their support network on medical conditions. Perform urgent and routine follow-up visits as determined by the participant's needs. Develop and implement a discipline-specific plan of care in collaboration with the interdisciplinary team, ensuring high-quality, value-based, and compassionate care. Identify participants' issues and determine priorities while collaborating effectively with other team members. Coordinate care for participants with a high level of complexity, ensuring seamless transitions of care and preventing unfavorable outcomes. Attend to Clinical Inbox and ensure that orders are completed in a timely manner and appropriate actions are taken. Document ordered procedures and care in various settings as required. Communicate participant changes to the interdisciplinary team, including post-procedure needs. Coordinate with the Complex Care Team on participant discharge and transition needs. Review results from medical tests and work with the provider on timely responses to participants, ensuring documentation and follow-up. Provide support for specific tasks as needed in support of the clinic, the Primary Care Provider, and the delivery of participant care.
