Tryfacta is seeking a CNA - LTC for our client in Burlington, VT 05401. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please apply now!

Location: Burlington, VT 05401

Duration: 13 weeks (possible extension)

Work Schedule: 5x8 day shift Shift: 40hr./wk.

Responsibilities for this position include, but are not limited to:

• Administer medications and treatments as prescribed.
• Perform patient assessments and monitor changes in condition.
• Provide nursing care for LTC, rehabilitation, and dementia residents.
• Maintain accurate documentation using PointClickCare (PCC).
• Collaborate with RNs, CNAs, physicians, and interdisciplinary teams.
• Ensure compliance with infection control and patient safety protocols.
• Deliver compassionate, resident-centered care.

To be considered for this position, you should have:

• Active LPN license.
• Minimum 1 year of verifiable travel experience required.
• BLS certification.
• Experience in long-term care, rehabilitation, or dementia care preferred.
• Experience with PointClickCare (PCC) charting preferred.

Important notes:

• Patient ratios: LPN: 1:18, RN: 1:18, CNA: 1:12
• Unit details: 4 units and average census: 95
• No 12-hour shifts.
• No RTO accepted at submission. Any RTO requests must be submitted after an offer and are subject to facility approval.
• No specific scrub color required; no jackets or hoodies on the unit.
• Public transportation (Uber, Lyft, Taxi, Bus) is available.
• Closest airport: Burlington International Airport.
