Physical Therapist - $20,000 Sign on Bonus & Relocation Assistance

Position Overview Physical Therapist – Home Health (Nicholas and Fayette County). Sign‑On Bonus: $20,000. Relocation Bonus: available on request. Primary Responsibilities Evaluate patient functional status and physical therapy needs; assist physician in developing the therapy plan of care. Provide services within the scope of practice defined by state laws, in accordance with the plan of care and in coordination with the health‑care team. Observe and report patient response to treatment and any changes in condition to supervising nurse and/or physician. Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using accepted standards of practice. Educate patient, family/caregivers, and health‑care team on the plan of care, functional limitations, and progress toward goals. Visit patients per Plan of Treatment, complete a progress note for each visit, and submit notes per LHC policy. Complete all patient evaluations and develop the PT plan of care within state‑specific guidelines. Report evaluation outcomes, goals, and projected frequency of care. Required Qualifications Current Physical Therapy licensure in state of practice. Current CPR certification. Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. Pay Range Annual Target Pay: $100,281 – $150,422. Hourly Pay Range: $48.21 – $72.32. Per Visit Point Pay Range: $57.85 – $86.78. Annual total cash compensation assumes full‑time employment (40 weekly hours) at full productivity. Compensation includes earnings from per visit point pay and hourly pay and may increase over time based on productivity and performance.
