Future Position: Advanced Practice Provider (Nurse Practitioner / Physician Assistant) - California We are seeking compassionate and skilled Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to deliver high-quality, in-home care to patients with multiple chronic conditions such as diabetes, hypertension, chronic kidney disease, heart failure, COPD, and depression.
