Care Mgmt Social Worker MSW

Overview Promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manage, support and develop comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Work to promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families. Responsibilities Ensure patient and family wellness, improve care outcomes, and facilitate access to hospital and community resources. Develop and support comprehensive transitional care plans for patients with complex psychosocial needs in Inpatient and Emergency Department (ED) settings, and collaborate with service team members, physicians, patients, external agencies and families to promote quality care and effective transitions. Qualifications Master’s degree in Social Work from an accredited social work program (Required) One year of experience in an acute or post‑acute setting (Required) Licenses and Certifications LMSW – Licensed Medical Social Worker, state licensure (Preferred) Accredited Case Manager (ACM) (Preferred) Certified Case Manager (CCM) (Preferred) Additional Licenses and Certifications Licensed Master Social Worker (LMSW) in the state of Georgia (Preferred) #J-18808-Ljbffr
