This Home Health LPN role is based in Northampton, Massachusetts, a lively New England community known for its welcoming atmosphere, walkable downtown, and rich cultural scene. The presence of nearby colleges contributes to an energetic, forward-thinking environment, while the town's historic charm and strong sense of community make it an appealing place to live and work.As a Home Health LPN, you will provide direct nursing care to patients in their homes throughout Northampton and nearby areas. You will work under the supervision of an RN and alongside an interdisciplinary team to ensure that each patient receives safe, thorough, and personalized care.Typical responsibilities include: Conducting focused nursing assessments during home visits and reporting changes in condition to the RN and provider Administering medications and treatments as ordered Providing wound care and dressing changes Assisting with disease management and symptom monitoring for conditions such as heart failure, COPD, diabetes, and post-surgical recovery Reinforcing patient and family education on medications, disease processes, home safety, and equipment use Documenting all care and patient interactions in the EMR in a timely and accurate manner Coordinating schedules to complete multiple visits per day while maintaining quality care standards Collaborating closely with RNs, therapists, social workers, and other team members to support comprehensive care plansA typical day in this role includes traveling to patients' homes in and around Northampton, completing scheduled visits, and providing one-on-one care that allows you to build strong, trusting relationships with patients and their families. Patient caseloads reflect a mix of adult and geriatric individuals, often with chronic conditions or recent hospital or facility discharges. You will have the opportunity to see the impact of your care directly in the patient's daily life, often helping them remain safely at home and maintain independence.Shifts are generally daytime and early evening hours, with some flexibility based on patient needs and team scheduling. Visit volumes and routes are designed to be manageable, allowing time for thorough assessments, interventions, and documentation.
