Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: CHM STILSON CENTER Duration: 13 weeks Shift: 0800-2030 Hours per Shift: 10 hours Experience: Minimum of 1 year of experience required License: State License required Certifications: BLS Certification required Must-Have: BLS Certification due by 8/28/2026 Competency Validation (optional) Learnshare Annual Education (optional) TRA Orientation due by 8/28/2026 Health Statement due by 8/28/2026 TB Test/PPD due by 8/28/2026 Annual TB Questionnaire due by 8/28/2026 Drug Screen due by 8/28/2026 Hepatitis B due by 8/28/2026 MMR due by 8/28/2026 Varicella due by 8/28/2026 Tdap due by 8/28/2026 Flu Vaccine due by 8/28/2026 COVID-19 Vaccine due by 8/28/2026 Annual TRA Evaluation due by 8/28/2026 Occupational Therapist - Skills due by 8/28/2026 Occupational Therapist - Test due by 8/28/2026 Unit Knowledge Exam due by 8/28/2026 OIG, SAM/GSA due by 8/28/2026 Reference Check due by 8/28/2026 Annual TB Screening due by 8/28/2026 Timekeeping Guidelines due by 8/28/2026 Client General Orientation due by 8/28/2026 Description: The position involves providing occupational therapy services to clients. You will assess clients' needs and develop individualized treatment plans. Collaboration with interdisciplinary teams to ensure holistic care is essential. Documentation of therapy sessions and client progress is required. Participation in professional development and continuing education is encouraged. Adherence to all health and safety regulations is mandatory. You will engage in community outreach and patient education initiatives.
