Nurse Practitioner

Nurse Practitioner (NP) or Physician Assistant (PA)Carease Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.Success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.We offer great compensation, flexibility, and a supportive clinical environment.Requirements:Active NP or PA license (in the state of practice)1+ year of experience in:Geriatric careDocumentation reviewExcellent communication, collaboration, and teamwork skillsBenefits:Hybrid Role!
