Monitor residents for changes in condition; perform routine assessments and document findings in the charting system. Assist with activities of daily living (ADLs), including hygiene, mobility, nutrition, and comfort measures. Support wound care, skin integrity, and pressure injury prevention per care plans. Collaborate with the interdisciplinary team to develop, implement, and update individualized care plans. Ensure safety, infection control, and patient rights are maintained; communicate changes to the care team promptly. Requirements Active LPN license in Rhode Island (or current eligibility to practice in RI) CPR certification (BLS) preferred Completion of an approved practical nursing program 1+ year of long-term care or skilled nursing facility experience preferred Ability to work a day shift (7:00 a.m. – 3:30 p.m.) and every other weekend; willingness to participate in a travel assignment Strong documentation and communication skills; competency with standard EHR systems Benefits and Support Weekly pay: $1,153–$1,206 (per week) Guaranteed hours: 37 per week Contract length: 10 weeks Travel assignment with ongoing assignment support from Healthforce Healthcare LLC
