Pay Salary based on experience plus superior benefits plan (pto, paid holidays, HSA, medical, dental, vision, pension and more) Job Description The Care Manager / Social Worker will provide care management services to a Medicaid program that helps older adults with disabilities receive care and support in their homes, instead of a nursing home. Duties to include: provide intake, assessment, and care management services to clients, which includes ongoing assessment for program eligibility, monitoring all aspects of care and client services (social and medical), and ensuring needs of clients and caregivers are met through a collaborative process.
