Qualified candidates will be considered for employment upon contract award.About The OpportunityJoin a collaborative care team as a Case Manager RN, coordinating care transitions and complex-case management for active-duty service members, retirees, and their families across the 6 MDG system of care.ScheduleFull-TimeMonday–Friday9-hour shifts, duty hours 0730–163060-minute unpaid lunchNot to exceed 40 hours per weekNo on-callResponsibilitiesAssess, plan, implement, and coordinate individualized case management plans for patients with complex medical or psychosocial needsCoordinate care transitions, referrals, and discharge planning across the interdisciplinary care teamIdentify and connect patients with appropriate community and military health system resourcesMonitor high-risk and complex patients to support continuity of care and reduce avoidable utilizationCollaborate with providers, utilization management, and behavioral health staff on shared casesDocument case management activity accurately within the electronic medical recordMinimum QualificationsU.S. Citizen, U.S. national, lawful permanent resident, or individual otherwise authorized to work in the United StatesAssociate's Degree of NursingCurrent, full, active, unrestricted Registered Nurse license in any U.S. state, territory, or the District of ColumbiaPrior clinical nursing experience; case management experience strongly preferredCurrent Basic Life Support (BLS) certification (or willing to obtain)Able to pass a Tier 1 (or higher) background investigation for facility and network access3 years in last 4 years as RN case manager, plus one of 8 named case-management certificationsPreferred QualificationsCertified Case Manager (CCM) credential or equivalentExperience in case management within a military treatment facility or TRICARE-network settingStrong care coordination, documentation, and interdisciplinary communication skillsWhy GiaCare?GiaCare is dedicated to supporting our nation's military by connecting exceptional healthcare professionals with military treatment facilities throughout the United States.
