ACT Care Coordinator - QMHP

Housing Stability SpecialistAssertive Community Treatment (ACT) teams work with individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions, utilizing a recovery based and trauma-informed perspective. ACT teams operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill-set by forging therapeutic relationships with community members. Interventions and services are typically provided in the community and require the use of a personal automobile and agency assigned cell phone.The Housing Stability Specialist is a Qualified Mental Health Professional (QMHP) who works within a multidisciplinary team to help individuals and families achieve housing stability. This position works in partnership with individuals to define and facilitate their desired life outcomes. Housing Stability Specialists deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain housing, locate and remove barriers to housing, apply for housing and housing programs, and coordinate ongoing rental assistance. Housing Specialists may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities as well as provide service coordination to obtain services related to employment, education, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need.Qualified Mental Health Professionals (QMHP) embrace trauma-sensitive and informed practices, provide Suicide Safer care, and foster a supportive and welcoming environment. QMHPs operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill set by forging therapeutic relationships with community members and/or stakeholders. Interventions and services can be both office- and community-based and may require the use of a personal automobile. Individuals served may be diagnosed with mental illness, cooccurring substance use disorders, chronic health conditions, and/or IDD. Staff members in this position are working under the supervision of a clinically licensed professional whoRegular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. Master's degree preferred.License:Required:Current and valid driver's license and a clean driving recordHours: Full-Time.Workdays: Monday - Friday.Workhours: 8:00 AM - 5:00 PM.Salary: $24.18- $25.39 an hour based on education, plus an additional 10% crisis stipend.Integral Care has proudly served Travis County residents for the past 50 years.
