We are seeking an experienced and compassionate Social Services Director to join our skilled nursing team!

The Social Services Director plays an important role in supporting our residents and their families throughout their stay. This position is responsible for addressing psychosocial needs, coordinating services, advocating for residents, and helping ensure a smooth transition through admission, care planning, and discharge.

Responsibilities

• Complete psychosocial assessments and identify residents' social, emotional, and psychological needs.
• Develop and maintain individualized care plans in collaboration with the interdisciplinary team.
• Participate in care plan meetings and communicate effectively with residents, families, and staff.
• Serve as an advocate for residents and help address concerns, needs, and grievances.
• Provide support and counseling to residents and families during adjustment to the facility and changes in condition.
• Assist with admissions, transfers, and discharge planning.
• Coordinate discharge services and collaborate with families, physicians, community resources, and outside agencies.
• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
• Assist residents and families with community resources and available services.
• Conduct follow-up with residents and families as needed to ensure needs are being addressed.
• Work closely with nursing, activities, therapy, dietary, administration, and other members of the interdisciplinary team.
• Maintain confidentiality and ensure resident rights are respected.
• Assist with compliance related to Social Services and applicable state and federal regulations.
• Help create a positive, respectful, and resident-centered environment.

Qualifications

• Previous Social Services experience in a skilled nursing, long-term care, assisted living, or healthcare setting preferred.
• Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field preferred.
• Experience with care planning, discharge planning, psychosocial assessments, and resident advocacy.
• Strong communication and interpersonal skills.
• Ability to work effectively with residents, families, and an interdisciplinary team.
• Compassionate, organized, and able to manage multiple priorities.
• Knowledge of long-term care regulations and resident rights preferred.
