Growing together.Work Schedule:There is an on-call rotation every 4-5 weeks, which runs Monday through Sunday and may include some holidays. The regular schedule is an 8-hour shift, Monday through Friday, between 8:00 AM and 6:00 PM.Location:19500 W IH-10, San Antonio, TX 78257Primary Responsibilities:Daily scheduling of all physician orders, new referrals, and schedules with appropriate field cliniciansReports new or pertinent patient information to field cliniciansSchedules per physician order all visits/tasks specific to each patient from the beginning to conclusion of directed patient care: supervisory visits, re-certification visits, discharge visits, transfers, and post-hospital visits and ensures accuracyUpdates clinician schedules frequently to remain in compliance with current physician ordersCommunicates all new physician orders and new referrals with contract therapy staffResponsible for checking email, Tiger Connect, team messaging system and voicemail to respond to time-sensitive messages from customers, leadership, lower-level managementPrepares clinicians schedules and assign clinicians to patients based on the frequency and expected duration of prescribed treatments and therapies per physician orderDemonstrates exceptional customer service to all patients, clinicians, and family membersParticipates with on-call rotation for home health approximately one week per month to include shared holiday coveragePerform general clerical duties in support of patient services.
