Job Description

South Mountain Restoration Center (SMRC) is an OMHSAS-operated long-term care facility located in South Mountain, Pennsylvania (Franklin County) . SMRC provides specialized long-term care services to residents with complex medical, behavioral health, and psychosocial needs.


SMRC is located in a rural area , and candidates should be fully aware of the location, commute expectations, and transportation requirements before accepting an assignment. Recruiters must confirm that candidates have reliable transportation and are able to consistently report to the facility for their scheduled shifts.


This assignment requires candidates who are dependable, flexible, and comfortable working in a specialized care environment.


Patient Population & Care Environment


Candidates should understand that SMRC is not a traditional long-term care setting. The resident population requires staff who are comfortable providing care to individuals with complex medical and behavioral health needs.


Residents may include individuals with:

• Significant medical care needs requiring skilled nursing support
• Behavioral health diagnoses and support needs
• Dementia and cognitive impairments
• Complex psychosocial histories
• Chronic medical conditions requiring ongoing monitoring and care
• Individuals transitioning from state-operated programs or other structured care environments

Staff assigned to SMRC must demonstrate:
