Nurse Practitioner

AlignMed Partners is seeking a compassionate and skilled Nurse Practitioner to join our Healthcare & Medical Services team in delivering high-quality, patient-centered care. In this role, you will provide comprehensive clinical services, including health assessments, diagnosis, and treatment for a diverse adult patient population in a collaborative, team-based environment. Working closely with physicians, nurses, and allied health professionals, you will manage both acute and chronic conditions, develop individualized treatment plans, and support preventive care initiatives that align with evidence-based best practices.
As a Nurse Practitioner with AlignMed Partners, you will conduct thorough patient histories and physical examinations, order and interpret diagnostic tests, prescribe and manage medications within your scope of practice, and offer clear education to patients and their families regarding diagnoses, treatments, and lifestyle modifications. You will document all encounters accurately in the electronic health record, ensuring compliance with regulatory standards, clinical guidelines, and organizational policies while maintaining strict confidentiality and adherence to HIPAA requirements.
You will play a key role in care coordination, collaborating with multidisciplinary teams to ensure timely follow-up, appropriate referrals, and seamless transitions of care. Your responsibilities will include participating in quality improvement initiatives, contributing to the development and refinement of clinical protocols, and supporting initiatives that enhance patient outcomes, operational efficiency, and the overall patient experience. Depending on clinic needs, you may also provide care through both in-person and telehealth visits, offering patients convenient access to services while maintaining high standards of clinical quality.
