Role Overview A Speech Language Pathologist (SLP) provides direct speech therapy rehabilitation to patients in a skilled nursing facility setting, collaborating with an interdisciplinary team to help patients achieve functional progress and improve quality of life. Key Responsibilities Assess, establish, and modify patient-specific, measurable, and functional therapy goals Implement and follow speech therapy treatment plans in accordance with laws and standards Chart accurately and utilize therapy software effectively Oversee and evaluate care provided by Speech-Language Pathology Clinical Fellows (CFYs), if applicable Participate in community health projects and maintain professional relationships Maintain confidentiality, adhere to policies, and demonstrate professionalism Assist in patient evacuation and demonstrate dependable attendance Communicate effectively within interdisciplinary teams Qualifications & Skills: Graduate of an accredited speech therapy program (MS or MA SLP) ASHA certification (CCC) or eligible for Clinical Fellowship Year (CFY); must maintain certification Active state licensure; CPR certification required Preferred: 1+ year experience in post-acute care Proficiency in Microsoft Office and strong communication skills Ability to make independent decisions, demonstrate good body mechanics, and adhere to safety standards Additional responsibilities include participating in community health initiatives, ensuring compliance with regulations, and providing excellent patient care in a supportive environment. #J-18808-Ljbffr
