Job Description Job Description Reliant at Home is seeking a Full-Time Home Health LVN to cover North and Northwest Dallas County and surrounding areas. Home health experience required. Participates in planning and implementing care in partnership with the patient, family, caregiver, and the RN. Provides services that are ordered by the physician as indicated in the plan of care. Provides patient, family, and caregiver education. Provides effective communication to the patient, family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes.
