Family Practice - Without OB Physician

We are partnering on a Market Physician Executive opportunity in South New Jersey . This innovative, value-based care organization is focused on transforming kidney and chronic care delivery through an in-home, team-based model.

The Market Physician Executive is a key clinical leader responsible for driving clinical strategy, overseeing interdisciplinary care teams, and improving patient outcomes across the market. This role combines leadership, population health, and direct patient care , and is primarily remote with local travel.

Position Details:

• Full-time, Monday Friday (8:30 AM 5:00 PM)

• Remote with local travel and occasional in-home patient visits

• Must reside in Camden, Gloucester, Burlington, Atlantic City, or Cumberland

Responsibilities Include:

• Provide direct and indirect patient care for complex, chronic populations

• Lead and support care teams (APPs, RNs, social workers, pharmacists)

• Drive outcomes including quality metrics, utilization, and total cost of care

• Oversee care plans, clinical pathways, and high-risk patient management

• Collaborate with community providers and health plan partners

Requirements:

• Board Certified in Family Medicine or Internal Medicine (other relevant specialties such as Geriatrics or Nephrology will also be considered)

• Active New Jersey license

• Leadership experience

• Experience in value-based care, Medicare Advantage, or managed Medicaid
