Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for current diagnosis, past medical history, precautions, and contraindications
• Establish measurable goals and develop a treatment plan for patient independence and function
• Regularly re-assess treatment effectiveness and modify goals or treatment as needed
• Communicate patient needs and progress to the treatment team and appropriate parties
• Consult with medical providers on patient evaluation, treatment, and progress
• Recommend adaptive equipment to improve function and incorporate it into the rehab program
• Maintain and submit required documentation in compliance with regulations
• Adhere to professional standards and legal regulations
• Supervise Physical Therapist Assistants and Aides
• Participate in internal staff development and maintain clinical skills through continuing education

Requirements of the Physical Therapist

• KY PT License, Compact License Accepted
• Experience with Home Care and Home Health
• Medicare Oasis experience
