Location: Vista, CA 92083 Schedule: Monday – Friday | 8:00 AM – 5:00 PM Requirements CNA Certification (Required) California Department of Public Health Home Health Aide (HHA) Certificate (Required) CPR & First Aid (AHA preferred) Minimum 1 year experience with elderly/frail population Valid California Driver’s License Basic computer skills (MS Office, Outlook, Excel) Role Overview As part of the PACE (Program of All-Inclusive Care for the Elderly) team, you will support care coordination efforts by ensuring home-based services are delivered efficiently and compassionately . You’ll work closely with RNs, physicians, social workers, and caregivers to provide a seamless care experience.
