Social Worker LMSW / Hospice - PRN

Role: Licensed Social Worker in hospice care, serving a designated territory including Bay, Saginaw, Arenac, Midland Counties, and surrounding areas. Responsibilities Conduct compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers. Provide ongoing emotional support, counseling, and grief assistance throughout the care journey. Identify and connect families with community resources, including financial aid. Guide families through advanced directives with sensitivity and clarity. Assist with facility placement when appropriate. Maintain accurate and timely clinical documentation at patient residences and agency records. Participate in interdisciplinary team and IDG meetings to develop holistic care plans. Manage caseloads while ensuring compliance with relevant laws, regulations, and policies. Contribute to on-call duties during evenings and weekends as needed. Qualifications & Skills Master’s degree from an accredited school of social work. State licensure and/or certification as required. Minimum of 2+ years in healthcare or social services, with hospice or palliative care experience preferred. Comfort with grief, loss, and end-of-life discussions. Strong knowledge of community resources and care coordination. Excellent documentation and compliance skills. Self-motivated with minimal supervision.
