This role provides skilled nursing care, after-hours triage support, and home visits for home health and hospice patients throughout the Albany and Linn-Benton area. Provide skilled nursing visits in patient homes. Complete admissions and follow-up visits as needed. Administer medications and treatments as ordered. Assess patient conditions and communicate changes to providers. Educate patients and caregivers on care plans and symptom management. Maintain accurate and timely documentation. Collaborate with physicians, case managers, and interdisciplinary teams. Qualifications Current Oregon LPN License required. Graduate of an accredited nursing program. Current CPR certification. 1–2 years of acute care nursing experience. Home Health or Hospice experience preferred. Valid driver's license, reliable transportation, and auto insurance.
