Physical Therapist - Days Schedule

Location: Lafayette, IN

Assignment Duration: This is a 13-week contract position.

Schedule: Days - 5x8, may be open to 4x10s, to be discussed in the interview.

Job Description: The Physical Therapist will manage therapy services according to established standards of care, focusing on evaluations, treatment plans, and patient interventions. The therapist will have the opportunity to work with a diverse patient demographic, including adolescents, adults, and geriatrics, addressing various orthopedic and neurological conditions.

Responsibilities:

• Perform evaluations and assessments of patients.
• Develop and implement individualized treatment plans.
• Manage a caseload, ensuring effective patient care.
• Document patient progress and modify treatment plans as necessary.
• Work collaboratively with other healthcare professionals to optimize patient outcomes.

Required Experience / Certifications / Licensure: Preferred candidates will have previous physical therapy experience and a valid state PT license. Experience with case management, screening, and building a caseload is essential.
