Work Environment

This position will be based onsite at our outpatient office in Paterson, offering a structured yet dynamic working environment. As a Nurse Practitioner, you will typically work from 9:00 a.m. to 6:00 p.m., with some evenings until 8:00 p.m. for patient clinics. Additionally, you may be required to participate in weekend community outreach events and health fairs. You can expect an average workweek totaling approximately 40 hours, where you will find a culture of collaboration and support among healthcare professionals.

Key Responsibilities

• Conduct comprehensive health assessments and screenings that consider the physical, emotional, and social factors influencing patient health.
• Diagnose and manage both acute and chronic medical conditions, implementing preventive care strategies tailored to patient needs.
• Develop and lead health education programs in collaboration with community organizations to raise awareness about health issues and promote healthy practices.
• Advocate for patients' rights to access healthcare services and provide culturally sensitive care that respects diverse backgrounds.
• Coordinate referrals and follow-up care to ensure continuity of treatment and support for patients with complex health needs.
• Maintain accurate medical records in compliance with regulatory standards, ensuring documentation supports best practices in patient care.

You will primarily serve a diverse patient population within the Paterson community, including individuals and families facing socioeconomic challenges. Your work will focus on underserved populations requiring tailored healthcare solutions. Qualifications

• Current certification as a Nurse Practitioner.
• Active state nursing license in New Jersey.
• Bachelor's degree in nursing; a Master's degree from an accredited program is preferred.
