Breast Health Navigator — Inpatient Care & Education Lead
Northwestern Medicine
Northwestern Medicine is seeking a Breast Health Navigator to oversee care of breast cancer patients in the West and Northwest Regions. You will work with the multidisciplinary team to establish an evidence-based continuum of care and optimize outcomes, including rounded patient assessments and discharge planning.
