Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Active PTA license in IL.

Assignment Details

• 36 hours per week.
• Shift hours: 8:00 AM to 4:30 PM.
• No on-call or weekend requirement.
• Coverage areas include Wabash, White, and Hamilton counties.
• Productivity expectation: 30 points.
