Care Coordinator II – Clinical Care TeamSea Mar Community Health Centers is seeking a Care Coordinator to support patients with chronic conditions and behavioral health needs. As part of the Clinical Care Team, you will participate in daily huddles, identify patient needs, close care gaps, and coordinate care at the point of service.Key ResponsibilitiesParticipate in daily huddles and identify gaps in careProvide brief interventions for chronic illness and behavioral health supportCoordinate care transitions after ER or hospital dischargeConnect patients to medical, behavioral health, dental, and community resourcesTrack and document patient progress in the EHRSupport quality improvement and clinical performance measuresAssist with 340B medication assistance program (site dependent)Provide excellent customer service and patient supportQualificationsLPN (ambulatory care experience preferred) or BA/BS/BSW in health-related field + 1 year community health experienceOR 4 years equivalent experienceStrong communication, organization, and patient engagement skillsKnowledge of chronic disease and behavioral health preferredBilingual English/Spanish strongly preferredValid WA driver's license and proof of insurance requiredCOVID-19 vaccination requiredMust obtain BLS CPR within 90 days of hireSea Mar is an Equal Opportunity Employer and offers a competitive benefits package.Starting Wage: $23.31 per hourWork Schedule: Monday – Friday (Can be edited based on need)What We Offer:MedicalDentalVisionPrescription coverageLife InsuranceLong Term DisabilityEAP (Employee Assistance Program)Paid-time-off starting at 24 days per year + 10 paid Holidays.We also offer 401(k)/Retirement options and an exciting opportunity to work in a culturally diverse environment.
