Hospice Nurse (On call / PRN)

Job Description Harmony Hospice Care – San Antonio Harmony Hospice Care is seeking dependable, compassionate, and organized PRN & On-Call Nurses (RN or LVN) to join our team. This is an opportunity for a nurse who believes in the heart of hospice care. Building meaningful relationships, supporting patients and families, and providing care with professionalism, consistency, and compassion. Position Type PRN / On-Call RN or LVN applicants welcome The Role Provide compassionate hospice care to patients in their homes, facilities, and surrounding communities Support patients and families with professionalism, empathy, and consistency Complete visits, documentation, and follow-up in a timely manner Collaborate with an interdisciplinary hospice team that values communication and teamwork Assist with routine visits, PRN visits, admissions support, or on-call support depending on experience and agency needs We are looking for nurses who are: Compassionate and patient-centered Reliable and accountable Organized and able to work independently Comfortable traveling throughout San Antonio and nearby surrounding areas Strong communicators with patients, families, and team members Committed to providing thoughtful and consistent hospice care Qualifications Current Texas RN or LVN license in good standing Hospice experience preferred, but not required for the right candidate Current driver’s license and reliable transportation Ability to document accurately and professionally What You Can Expect Flexible PRN scheduling Supportive leadership and team environment Meaningful work that makes a real difference A workplace where compassion, professionalism, and teamwork matter Our Goal We are building a team of nurses who take pride in doing hospice care the right way — with heart, professionalism, and respect for the patients and families we serve.
