Nurse Practitioner / Physician Assistant (Part-time)

Position Information Location: Eustis, FL Role: Physical Medicine & Rehabilitation (PM&R) NP/PA Clinician – post‑acute skilled nursing facility (SNF) care. Hours: 16‑hour part‑time W2 (8‑hour shift, flexible within 9AM–4PM on weekdays). Represent CRC in the facility, providing clinical leadership and collaborating with the interdisciplinary care team. Collaborate with PCP teams on management of conditions common in SNFs, such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries. Work with PT, OT, and SLP teams to improve patient function and quality of life. Track therapy patients’ progress and address barriers to therapy (pain, spasticity, neurogenic bowel/bladder, etc.). Participate in multidisciplinary meetings, bridging the gap between therapy and internal medicine. Engage in ongoing education, including monthly didactic lectures and regional meetings. Ideal CRC Clinician Passion for post‑acute healthcare and geriatric patients. Preferred experience in SNF or post‑acute setting. Confidence in autonomous practice.
