LPN - The Fountains at Atco At The Fountains at Atco, a continuing care retirement community, we are seeking a dedicated and compassionate LPN to join our team of healthcare professionals. As a valued member of our care community, you will play a vital role in providing exceptional care to our residents. Responsibilities: Provide high-quality nursing care to residents, including administering medications, wound care, and other treatments as needed Conduct initial and ongoing assessments of residents' physical and emotional needs Develop and implement individualized care plans in collaboration with the interdisciplinary care team Monitor residents' conditions and report changes to the charge nurse or other healthcare professionals Maintain accurate and detailed records of resident care and services provided Collaborate with other healthcare professionals to ensure seamless transitions and coordination of care Requirements: Current LPN licensure in the state of New Jersey Minimum 1 years of experience in a long-term care or healthcare setting Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment Commitment to providing exceptional resident care and supporting the mission of The Fountains at Atco What We Offer: 12 hour shift 7a-7p and 7p-7a #J-18808-Ljbffr The-Fountains-At-Atco
