LPN Case Manager

The Licensed Vocational Nurse (LVN/LPN) Case Manager supports the Transitions of Care program aimed at reducing 30-day hospital readmission rates. Working under the direction of an RN Care Manager, the LPN assists in implementing care plans through appointment scheduling, biometric data monitoring, patient education, home visits, and connecting patients with community resources. This role requires strong communication, documentation skills, and the ability to work independently within a multidisciplinary care team. Travel to patient homes, hospitals, and provider offices is required, but this is not a hands-on nursing position. Responsibilities: Required Skills & Certifications:

• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of practice.
• Basic Life Support (BLS) certification.
• At least 1 year of experience in home health, case management, or related nursing field.
• Strong communication and documentation skills, including proficiency with electronic health records (EHR).
• Ability to work independently and manage multiple patients effectively.
• Access to reliable transportation for travel to patient homes, hospitals, and provider offices.

• Experience or clinical knowledge in managing patients with congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), and multiple complex comorbidities.
• Familiarity with care coordination and community resource navigation.
• Experience working in a multidisciplinary care team environment.

• This is a mobile role requiring travel to patient homes, hospitals, and provider offices.
• The position is not hands-on nursing; it focuses on care coordination, education, and monitoring.
• Participation in virtual care team rounds with social workers, pharmacists, and other professionals is required.
• The role supports a 30-day transitions of care program aimed at reducing readmissions.
• Reliable transportation and the ability to work flexibly during the day are essential.

• Full-time position with a guaranteed 40-hour workweek.
• Regular schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m.
• No weekend shifts required.
• Floating between multiple locations including home visits, hospital, and provider offices.
