Nurse Practitioner - Hybrid Role (NP/PA) - Gastonia, NC

Carease Health - NC is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a key member of our team, you will build strong relationships with the facility, maintain a patient-centered approach, and be a collaborative team player.

About the Role

As a Nurse Practitioner (NP) or Physician Assistant (PA) with Carease Health - NC, you will:

• Provide high-quality, patient-centered care to residents in skilled nursing facilities

• Conduct on-site visits and remote consultations, supported by a dedicated scribe team for efficient documentation

• Build strong relationships with facility staff and residents

• Maintain a patient-centered approach and collaborate with the care team

• Participate in continuous quality improvement initiatives and contribute to the growth and development of the organization

Requirements

We are seeking a candidate with:

• Active NP or PA license (in the state of North Carolina)

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
