Description

• Conduct thorough evaluations for patients in inpatient or outpatient settings and tailor individualized rehab plans
• Administer targeted physical therapy techniques that emphasize mobility, strength, balance, and overall function
• Collaborate closely with nurses, doctors, case managers, and other rehabilitation professionals to ensure the best patient outcomes
• Guide patients and their families in understanding therapeutic objectives, discharge planning, and essential home exercises
• Keep meticulous records of evaluations, treatments, and patient progress in compliance with all facility and regulatory standards
• Engage in interdisciplinary care rounds and conferences to promote seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

• Current state licensure as a Therapist
• CPR certification preferred
• Any specialized certifications such as for neurological or geriatric care are an advantage but are not mandatory

Skills

• A minimum of prior experience is preferred
• Ability to address complex medical and mobility challenges
• Excellent communication and teamwork skills
• Comfort in utilizing digital charting and clinical software systems
