Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on patients' needs, goals, and physician direction. This role is responsible for the delegation and direction of therapy services, ensuring functional treatment programs are designed to facilitate effective patient return to community activities. The therapist will also serve as an educator to patients, families, students, and coworkers while maintaining adherence to the organization's Mission Statement and Core Values.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop treatment plans focused on patient progress.
• Document medical records clearly, accurately, and in compliance with all relevant guidelines and regulations.
• Participate in the student program, providing a positive learning environment and supervision for students.
• Create an individualized professional development plan, pursuing ongoing professional growth and continuing education.
• Adhere to state practice acts and maintain professional licensure in good standing.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable licensure in the state of practice.
• Previous experience with the agency is highly desired.

Schedule

This position operates on a 5x8.0 schedule.
